Mail your travel companion confirmation page, with a copy of receipt for purchase of the television to: MDEA Promotions, 1 Spectrum Pointe, Ste. 330, Lake Forest, CA 92630 4. Limit one Companion Ticket per household/family/person. 5. Qualifying applicants will receive a redemption code and redemption instructions by mail. Please allow 6 to 8 weeks for delivery of the Companion Ticket. 6. Terms and Conditions 1. The recipient of this certificate is entitled to purchase one adult round-trip airline ticket at the coach-class fare stated in this offer and receive a Companion Ticket. 2. Certificates are transferable only prior to submitting the Registration Form. 3. This program is valid only between the cities at the fares stated in the Companion Ticket certificate that will be mailed to you. 4. The person whose name appears on the Activated Companion Voucher must be one of the travelers. Both passengers must travel together on the exact same itineraries. 5. All tickets purchased are non-refundable, non-endorsable and non-transferable. Companion ticket available only in conjunction with purchase. Airlines impose fees to all changes on all tickets after they have been issued. These fees apply to both the paid ticket and the Companion ticket. 6. Reservations are generally accepted within six (6) months of travel but must be purchased at least fourteen (14) days in advance and may require a Friday or Saturday night stay. Maximum stay is thirty (30) days but minimum stay requirements vary. 7. All reservations must be made through Lifestyle Vacation Incentives (LVI). Airline participation varies from market to market, so selection of the airline is at the sole discretion of LVI based on availability. 8. The Companion Ticket applies to the base fare only. Taxes, passenger facility charges, segment fees, governmental fees, fuel surcharges and international departure fees, if applicable, will apply to both tickets. 9. Lower fares may be available in certain markets. Customers should consult with LVI agents to determine the most economical fare for their itinerary. 10. This offer may not be used in conjunction with any other discount or program, and frequent flier mileage accrual may not be applicable. 11. This certificate is published to share information about terms, conditions and use of the program and does not guarantee travel. Only the Activated Voucher issued by LVI after program registration will be valid for travel and will carry an expiration date. 12.
